Vanguard Health nurses provide non-critical care that does not need to be performed in a hospital setting. This could be short-term follow-up treatment after hospital discharge or long-term care for clients who require help with medications, diabetes, asthma or emphysema management, wound management and chronic conditions. 

Vanguard Health staff work responsively, in partnership with clients and their families to accomplish the individual’s goals and to find purpose in their lives.

Our nurses provide non-critical care that does not need to be performed in a hospital setting. This could be short-term follow-up treatment after hospital discharge or long-term care for clients who require help with:

  • Management of medications;
  • Asthma or emphysema management;
  • Pressure Care and Wound management;
  • Chronic conditions including diabetes;
  • PEG, continence, ventilator, bowel and stoma care; and
  • Assessments – Continence, holistic care plans.

Clients who live with a disability can receive support from an experienced Vanguard Health home care nurse.  We provide assessment by a qualified nurse on what services and level you need. We offer training by our nurse to enable you, someone else or a support worker to undertake supports like medication prompting and simple wound care.

Our experienced team can provide ongoing nursing and assistance with complex support needs.
Service provision conforms with the NDIS price guide, capacity building and assessment guidelines and enables you to access multiple support services for your clients.

We verify our nurses' educational credentials and registration, to ensure every nurse is compliant with AHPRA standards and working within their scope of practice. Our nurses maintain qualifications and registrations necessary for them to lawfully carry out their role.  All Vanguard Health nurses are carefully screened with criminal background checks and vulnerable sector checks.
